Your 2022 income sets your 2024 premium
Medicare uses the tax return filed two years earlier, not current income. The 2024 surcharge was set by the 2022 return, which is already filed.
The return filed for the current year sets the premium two years out. Conversions, sales and large withdrawals raise MAGI in the year they happen, and that year is the one a future surcharge is calculated from.
The tiers step. One dollar over a threshold moves the whole year into the next tier, so a few hundred dollars over a line can cost four figures. The table shows remaining headroom in dollars rather than just the bracket you are in — and what each crossing costs is worked out separately.
